چکیده

Abstract Introduction: The diagnosis of thyroid tumors arising from follicular cells that are encapsulated/well delimited and noninvasive is a challenge. When unequivocal nuclear alterations present, the final can range neoplasm with papillary-like features (NIFTP) classical/encapsulated papillary cancer (PTC), including aggressive subtypes, to poorly differentiated carcinoma. As proposed recently, presence papillae in absence exuberant (nuclear score 2), given other criteria for NIFTP met, may not be sufficient PTC. This condition called “noninvasive encapsulated RAS-like tumor” (NEPRAS), whose nature would borderline malignant. Revising our cases > 1 cm were diagnosed previously as PTC noninvasive, we found three NEPRAS. We now revised well-differentiated tumor uncertain malignant potential (WDT-UMP) because considered on occasion. Case: In 29-year-old euthyroid male patient single nodule fine-needle aspiration had revealed indeterminate cytology, measuring 3.2 was reclassified WDT-UMP For this diagnosis, met following criteria: encapsulation or clear demarcation, no vascular capsular invasion, papillae, < 30% solid/trabecular/insular growth pattern, necrosis high mitotic activity, 2. BRAFV600E mutation absent. continues show signs recurrence 7 years after lobectomy. Conclusion: Despite some (encapsulated PTC) (NEPRAS). proposal result change management, same implications those seen variant NIFTP. References: Ohba K et al. Programmed Death - Ligand 1 Expression Distinguishes Invasive Encapsulated Follicular Variant of Papillary Thyroid Carcinoma from Noninvasive Follicular Thyroid Neoplasm with Papillary-like Nuclear Features

BACKGROUND The noninvasive Encapsulated follicular variant of papillary thyroid cancer (EFVPTC) has been reclassified as Noninvasive follicular thyroid neoplasm with papillary-like nuclear features (NIFTP) without a significant risk for malignant behavior. However the evaluation remains a challenge for clinicians. We sought to determine whether programmed death-ligand 1 (PD-L1) expression may s...

Encapsulated papillary thyroid carcinoma, follicular variant: a misnomer.

Papillary thyroid carcinoma (PTC) has long been diagnosed based on its unique nuclear features (PTC-N); however, significant observer discrepancies have been reported in the diagnosis of encapsulated follicular patterned lesions (EnFPLs), because the threshold of PTC-N is subjective. An equivocal PTC-N may often occur in non-invasive EnFPLs and benign/malignant disagreements often create seriou...

Warthin-like tumor of the thyroid gland: an uncommon variant of papillary thyroid cancer.

Several variants of papillary thyroid cancer have been described, including, most recently, Warthin-like tumor of the thyroid gland. To bring attention to this uncommon variant, we review previous reports on this entity and we add 5 new cases to the literature. We retrospectively reviewed the records of all patients who had undergone thyroidectomy at our institution during a 7-year period. Amon...

Noninvasive follicular thyroid neoplasm with papillary-like nuclear features: a single-institutional experience in Japan.

Noninvasive encapsulated follicular variant of papillary thyroid carcinoma (PTC) was reclassified as noninvasive follicular thyroid neoplasm with papillary-like nuclear features (NIFTP). To date, no studies on NIFTP have been reported in Japan. This study aimed to evaluate the clinical, cytological, and pathological findings of 54 cases of NIFTP from a single center in Japan, and compare them w...

Noninvasive Follicular Thyroid Neoplasm with Papillary-Like Nuclear Features: An Evidence-Based Nomenclature Change

A consensus panel recently used clinical evidence and pathologic parameters to rename noninvasive encapsulated follicular variant of papillary thyroid carcinoma to noninvasive follicular thyroid neoplasm with papillary-like nuclear features (NIFTP) to better reflect the indolent course of this tumor. NIFTP has stringent histopathologic diagnostic criteria established by the panel, including pap...
